Our Office

The Wolcott Health Care Professional Center
A two story contemporary professional building located at 464 Wolcott Road is the home of Dental Health Associates, PC. We are on the upper level. The rear entrance of the building, shown left, has ample parking as well as easy access for the handicapped. An elevator found immediately to the right upon entry here, will bring you to the front door of our office. We can also be accessed from the front of the building and are located at the very end of the hallway on the left hand side. Public rest rooms are available immediately to the right upon entering the front or Wolcott Road entrance.

A guideline for fees involving cosmetic services......

Approximate fees for the cosmetic procedures discussed are as follows:

• » Bleaching $295 (includes upper and lower trays and bleach kit)
• » Composite Veneers $350 to $650 per tooth dependant upon complexity
• » Porcelain Veneers $750 to $1200 per tooth dependant upon complexity
• » Porcelain Crowns $900 to $1200 dependant upon type of crown

Payment Policies

As you visit our office requesting dental care you undertake a personal obligation and responsibility for your account. Our office policy is to request payment from you as services are rendered. In an effort to keep dental costs down, while maintaining a high level of professional care, we have established the following payment plans for the use of our patients:

• 1. We accept personal checks, bank checks and cash
• 2. We accept Master Card, Visa American Express and Discover
• 3. We offer a new line of instant credit called Care Credit, a credit card for dentistry. Care Credit offers payment plans extending 12 to 48 months more depending upon the amount borrowed.
• 4. We offer a 10% discount to our senior citizens on non-laboratory involved procedures.
• 5. We offer a 5% discount for prepayment or payment at time of all major treatment.

Insurance

DHA participates directly with several insurance companies: Blue Cross of CT, BlueCross Federal Plan, and Delta Dental USA and others on an ever-changing basis in the volatile Insurance market. We accept payment as determined by these insurances. If you have other dental insurance available we will help you maximize your benefits. Please bring your insurance benefit booklet. Your portions of the fees will be computed at the time of your appointment and this amount is due at the completion of that appointment.

Directions to the Wolcott Health Care Professional Center

From I-84 West: Take exit 25 (Scott Road). At end of exit take right. Take left at next light onto East Main Street. At next light take right onto Frost Road. Continue through next light (crosses Meriden Road intersection). When road splits, bear right onto Woodtick Road. Take left onto Sharon Road. At top of hill take right onto Wolcott Road (Route 69). Proceed approximately 1.6 miles to Dental Health Associates on right in Community Medical Specialists building (#464).

From Route 8 South: Take exit 31 and follow directions for 84 East.

From I-84 East: take exit 23 (Wolcott/Prospect). Follow to Hamilton Ave. Exit (Route 69N). At end of exit take right. Go straight through next 2 lights (crossing over Hamilton Avenue and East Main Street). At third light take left onto Manor Avenue. At next light turn right onto Wolcott Road. Follow straight through 8 lights for approximately 3.2 miles (passing the old Naugatuck Valley Mall on right). Dental Health Associates is on right in community Medical Specialists building (#464).
